12 teams of Japan Soccer League division-1 and 20 representative teams of nine regions are qualified for the competition.

If two teams are level at the end of regulation time, extra time shall be played.
If the match is still undecided, the winning team (in the final, the winner) shall be decided in a penalty shoot-out.

The winner, Matsushita, withdraws.
The moved-up team, Nissan, shall be eligible to play in Asian Cup Winners Cup 1991/92.
